There are numerous mistakes on this prescription · Wrong name and date of birth…..remember that if you get this wrong in the WriSKe then subsequent answers will score zero (this is information that is available to you already on the LSE, but worth re-reading, and worth bearing in mind if you have trouble with spelling etc. Free marks!!) · Little detail on allergies and no signature · No times on the once only drugs · Micrograms not written out correctly....(remember no mu and not mcg) · Trade name (should be co-amoxiclav) · Penicillin prescribed even though penicillin allergy documented · Unnecessary use of the decimal point on the dose. Should be 625mg not 0.625g · Correction….should be a single line and initialled at least but better to re-write · Illegible signature and no name · And to finish off: · Co-codamol wrong on multiple levels…double paracetamol prescription, doesn’t specify which sort of co-codamol, PRN is a bit unclear, poor signature |
